اختيار العرض/ المرض الجسدي بين أنماط السلوك، بروفيل الشخصية، الخصوصية والبنية السيكوسوماتية

The purpose of this article is to present theoretical models and clinical results that explain the reasons of choosing the organ /somatic disease. Some authors have explained it through the types of behavior / personalities ("A","C", "D") according to the field of health psychology. while others have given explanations that have marked the history of psychosomatic thought, which has emerged above all with the work of Alexander, Dunbar, and Marty. Through the scientific literature provided on the subject, personality traits, psychological structure alone are not sufficient to explain the choice of organ / somatic disease, but by their contributions with other factors: genetic, biological, physiopathological, psychic, emotional and social… because human beings are biopsychosocial and must be approached from an integrative-holistic perspective. the objective should not be limited to the classification of types, but rather to take them as preventive measures by avoiding pathogenic factors and favoring salutogenic factors

تهدف هذه المقالة إلى عرض النماذج النظرية وبعض النتائج الاكلينيكية حول الأسباب المفسرة لاختيار العرض/المرض الجسدي. فهناك من سعى لتفسيرها من خلال انماط السلوك أو أنماط الشخصية ("أ"-"ج"-"د") حسب مجال علم النفس الصحة، ومنهم من قدم تفسيرات ميزت تاريخ الفكر السيكوسوماتي من خلال ادراج دينبار لمفهوم الاستعدادية، وطرح ألكسندر لنظرية الخصوصية، ومفهوم البنيات الاساسية لمارتي. يتضح من خلال تحليل ما يتوفر من ادبيات وما توصلت إليه نتائج الدراسات حول الموضوع، بأن السمات الشخصية للفرد وحدها أو بنيته النفسية غير كافية لتفسير اختيار العرض أو المرض الجسدي، بل في اشتراكها بعوامل أخرى؛ وراثية، بيولوجية فيزيوباتولوجية، وجدانية، انفعالية، اجتماعية... على أساس أن الانسان وحدة بيونفسواجتماعي يجب تناوله في شموليته بنظرة تكاملية. فهي. وتعتبر بالتالي كعوامل مساهمة في ظهور العرض الجسدي واختيار أعراضيته. لا يتوقف الهدف عند حدود التصنيف وتحديد الأنماط، بل بوضع أسس الوقاية للتقليل من احتمالية الإصابة المرضية بتجنب عوامل الهشاشة الممرضة المهددة للصحة الجسدية وتشجيع عوامل الحماية التي تعزز الصحة الجيدة.

Cet article se propose de présenter des modèles théoriques et résultats cliniques qui expliquent le choix de l'organe/maladie somatique. Certains auteurs l'ont expliqué à travers les types de comportements/personnalités ("A","C","D") selon le domaine de la psychologie de la santé. Tandis que d'autres ont donné des explications qui ont marqué l'histoire de la pensée psychosomatique qui s’est surtout imposée avec les travaux d'Alexander, Dunbar, et Marty. À travers la littérature scientifique fournie sur le sujet, les traits de la personnalité, la structure psychologique ne sont pas à eux seuls suffisants pour expliquer le choix de l'organe/la maladie somatique, mais par leurs contributions avec d'autres facteurs : génétiques, biologiques, physiopathologiques, psychiques, émotionnels et sociaux… du fait que l’être humain est biopsychosocial et doit être approché dans une perspective intégrative-holistique. L’objectif ne devrait pas se limiter à la classification des types, mais plutôt à les prendre comme mesures de prévention en évitant les facteurs pathogènes et favorisant les facteurs salutogènes.
